1. Patient Form to complete online
   • If you are a NEW Patient, please select the office location and complete your Patient Information, Medical History and HIPAA forms

   • If you are an EXISTING Patient, please update your Medical History from home before your appointment

   • ALL Patients will be asked to complete our COVID Disclosure Form

2. Please bring a mask or face covering to be worn when not being treated
   • (i.e. walking through the office, using the restroom, checking out)
3. To check-in, please call our office number 
   • or reply to the appointment reminder text when you arrive 
4. We will call you to come in from your car
   • or we will come out to get you with your name on a dry-erase board
   • we ask that guardians and caretakers please wait in their vehicles while patients are being treated
